When a set has an entry as above how do we proceed? Do I leave the slot empty? Do I type in Not Issued? Do I delete that card number completey and add a note to the set? And if so under release notes or user comments? Thanks.

There have been plenty of skip number sets. And of course Topps no longer has a number 7 in their baseball sets. And there are a whole lot of skip number insert sets.  None of them have an empty slot(s) or notes. So I'd say just leave out the number.

Agreed, we should not have listings for nonexistant items.

Funny thing, I ran across this same situation in a set that I added scans for recently.  The checklist that was in the database showed two cards with the name "Not Issued", but my factory set included those two cards.  That was amusing to upload scans for cards described as Not Issued...LOL!  Of course I corrected the listings, but it would have been just as easy to insert the cards into the set if they had been left out to begin with.
