 Two items have come up and I need some instruction please. When submitting a new checklist, if there is a professional player as well as a college/minor league player on the same card, in their respective uniforms, is it ok to list both of the teams in the team title, separated by a "/" of course?

For your first question, it depends on which teams and logos are listed on the card.  The uniforms are irrelevant unless there are no teams listed on the card at all.

For your second question, you would list the initials in the CardNum field.
