I bought this on Ebay about ten years ago, and I seem to recall it stumping me even then. It may have been a stadium giveaway..

It was a stadium giveaway. 2005 Topps Poland Spring New York Yankees. I have seen a listing for an O'Neill(matches this card. Also an ebay listing for a 2004 don mattingly yankee stadium giveaway poland spring 3 of 4 but can't find the listing when I go to listing page.
