Took some sleuthing but I found the card on COMC and traced it to this set here: 2003 Upper Deck - NL All-Star Swatches. The card numbers were formatted incorrectly so I fixed them to add the "ASN-" prefix seen on your card, which is right here, and could definitely use your scans if you can fix the cropping a bit!

Thanks for adjusting the card numbers. I noticed the AL Swatches could use the same treatment. "ASA-" for those cards if you're feeling generous with your time.
