I've been trying to figure this out for a while now with no luck. Does anyone know where this card comes from or what card set or how it was obtainable? It is slightly smaller in size than a standard trading card and appears to be from 1997 and possibly some sort of Chromium type set.

If I remember correctly (I had a couple in my collection for a long time) They were comic shop givaways from the distributor. In my local comics shops case I think it was included with the monthly distributor pre order book. There was not a set just the stand alone ID card.

That is what it is a card shop giveaway promo. I think at the time Marvel was launching one of their huge multi-title cross-over events that involved The Avengers.
I had one (It is still around somewhere I think but in beat up shape). I had taped a photo of me (from an old student ID card) onto it and kept it in my wallet for a period of time (thus the beat up shape). After several years the photo tore or wore off of the card so I had to take the rest of it off. UhOh I might have gotten rid of it.
