My guess is that it's from this set, but we don't have a checklist here yet. Does anyone have one?

Wasn't there some kind of controversy about the legitimacy of Rize leading to it not being in the Database?

I could be misremembering all of this... Leaf was hired to print these cards, but I think they disowned all connection with these cards. Which begs the question why they added their copyright on it. I think they were removing any eBay auction that said "Leaf Rize" for a while.
