In the 1991 Topps set how was these inserted?

Was it 1 per cello pack?

or 1 per pack

I can confirm that the set was only a mail in offer. My buddy and I opened up three rak cases (9 rak boxes-216 rak packs total) and not a single glossy All-Star card was anywhere in 216 Rak Packs. In previous years they had been available 1 card per Rak Pack. In 1991 they were only available via a mail in offer, and in 1992 (and beyond) they were discontinued.
