ComposerMike. Forgive my ignorance but I don’t know what an IR is. I just provided the missing information. Another member just updated the checklist for me. I just joined to site today specifically to help complete this checklist after years of chasing it with almost zero info to go on. 

IR = Inaccuracy Report - on the checklist page, there is a spot that says "See any inaccuracies?" with a link to click to make that report. There are also links on any card's page if there is a mistake on the card. For what you're doing, entering it at the checklist level would be ideal.
