Looks like the rest are shown as being already ripped away from the checklist portion.

I guess it depends on what we're going call the definition of it as a "card".

I would treat it as other perforated "panels". Add an Insert and Related for the "complete panels". Keep the base set as the individual (separated) cards. And the checklist card should probably be added as an NNO 25th card on the checklist.
