The upper left corner logo is Collector's Edge 95. It seems 1995 Collector's Edge Football was the only "Edge" set that year. The inserts section has some "Quantum Motion" parallels or inserts. This seems to be some kind of exchange offer for the quantum parallel of cards or chance to get one.
