Well, my last request for help didn't get much help before flying off the boards, so let's try this one. It's from an SGA, apparently sponsored by Papa John's from the 2003-04 season. It's perforated (I accidentally cropped that out a little on the front image), and I believe came in six-card panels, although it may have been 9 - I really don't remember. I used to have more than just this one card, and still might, but this is all I've come across while inventorying basketball so far. That said, it's not on here, nor COMC, nor does a Google search turn anything up.
Would anyone happen to have a lead on a checklist for this set?
