Go back to the set's page and look on the left pane. You'll see a "See any inaccuracies?" link. Click that and enter all the info needed (number, name, team, etc.) and a link to this form thread as it contains an image.

I still see the glare of the sleeve.
1 - is this a scna done on a scanner (not a phone/mobile "scan app")?
2 - did you remove it from the plastic (as requested) before scanning it?

I don't own this card. Only photo I could find of it. I know it's not the best because of the glare
